What Is BMI?
BMI, or Body Mass Index, is a numerical value derived from a person's weight and height, used as a screening tool to categorize body weight relative to height at a population level. The formula divides weight in kilograms by height in meters squared (kg/m²). In US units, it uses pounds and inches with a conversion factor of 703. The result places a person into one of six WHO-defined categories: Underweight (under 18.5), Normal weight (18.5–24.9), Overweight (25–29.9), and three classes of Obesity (30 and above).
BMI was developed in the 19th century by Belgian mathematician Adolphe Quetelet as a statistical tool for studying populations — not as an individual health diagnostic. Despite this origin, it became the most widely used screening tool for assessing body weight because it requires no equipment beyond a scale and a measuring tape, and it correlates reasonably well with body fat at a population level.
However, BMI is a deeply flawed metric for individual health assessment. It cannot distinguish between muscle mass and fat mass — a muscular athlete and a sedentary person can have the same BMI while having drastically different body compositions. It does not account for age, sex, ethnicity, or where fat is stored on the body. It regularly misclassifies athletes as overweight and some individuals with high body fat as normal weight. Medical and public health organizations increasingly recommend using BMI alongside other measures — waist circumference, body fat percentage, and metabolic markers — rather than relying on it alone for health decisions.

Note for Asian populations: The WHO and many Asian health organizations use different BMI thresholds for Asian adults because health risks associated with excess body fat occur at lower BMI levels. Many guidelines recommend:
- Normal: BMI 18.5–22.9 (vs 24.9 in Western guidelines)
- Overweight: BMI 23.0–27.4
- Obese: BMI 27.5+
Why BMI Has Significant Limitations
BMI was developed in the 19th century by Belgian mathematician Adolphe Quetelet as a statistical tool for studying populations — not as an individual health diagnostic. Its limitations are well-documented:
1. Cannot distinguish muscle from fat: A competitive athlete with very low body fat and high muscle mass may have a "overweight" or even "obese" BMI. Many NFL players, bodybuilders, and endurance athletes are "obese" by BMI despite having excellent cardiometabolic health. Conversely, a sedentary person with low muscle mass and high body fat percentage may have a "normal" BMI while actually carrying unhealthy levels of fat.
2. Ignores fat distribution: Where fat is stored matters enormously for health risk. Visceral fat (around abdominal organs) is far more metabolically harmful than subcutaneous fat (under the skin). Two people with identical BMIs can have very different health risks based on fat distribution. Waist circumference and waist-to-hip ratio are better predictors of cardiometabolic risk than BMI.
3. Age and sex differences: Older adults often have lower muscle mass and higher fat mass at the same BMI as younger adults, making BMI less accurate as a health predictor for older populations. Women generally have higher body fat percentages than men at the same BMI.
Better alternatives or complements to BMI:
- Waist circumference: Men >40" and women >35" indicate elevated abdominal obesity risk
- Body fat percentage: Measured by DEXA scan, hydrostatic weighing, or bioelectrical impedance
- Waist-to-height ratio: Should be less than 0.5 for most adults
- Metabolic health markers: Blood pressure, fasting blood glucose, LDL/HDL cholesterol, triglycerides
Frequently Asked Questions
Is BMI accurate for children? Children's BMI is interpreted differently from adults. Rather than fixed categories, children's BMI is compared to sex- and age-specific growth charts as a percentile. A child at the 85th–95th percentile is "overweight"; 95th percentile and above is "obese." Because children's body composition changes with development, adult BMI thresholds do not apply.
Does BMI predict life expectancy? At a population level, BMI is associated with mortality risk — very high and very low BMI are both associated with higher mortality. But this relationship is confounded by many factors: fitness level, metabolic health, smoking status, and socioeconomic factors. A fit person at BMI 27 likely has better health outcomes than an unfit person at BMI 23. Cardiorespiratory fitness is a more reliable predictor of all-cause mortality than BMI in most research.
What BMI do insurance companies use for rate calculations? US health insurers (for employer plans under the ACA) can use BMI as one factor in wellness programs and premium surcharges. Life insurers often use BMI as part of their underwriting — BMI above 35–40 can result in higher premiums or coverage restrictions. These are policy considerations independent of the scientific limitations of BMI.
Can I be healthy outside the "normal" BMI range? Yes. Many people maintain excellent metabolic health with BMI in the 25–30 range (especially if muscular), and some people in the "normal" range have poor metabolic health. The concept of "metabolically healthy obesity" — being obese by BMI but having normal blood pressure, glucose, and lipids — is recognized in research, though long-term health trajectories are complex. BMI alone is insufficient to assess individual health.
How much does BMI change if I lose 10 pounds? Change in BMI from weight loss = lost weight (lbs) × 703 / height (inches)². For a person 5'7" (67 inches): losing 10 lbs changes BMI by 10 × 703 / 67² = 7,030 / 4,489 = 1.57 BMI points. So dropping from BMI 27.5 (overweight) to 25.9 — still overweight but closer to normal. Losing 20 lbs would drop BMI by 3.14 points.
